| It was my pleasure to attend the
2006 annual educational meeting of the ISRC titled Confluence Conference
II in Lewiston on September 14-15. The title relates to the fact that Lewiston
is at the confluence of two of the State's great rivers, the Clearwater
and the Snake. President Chad Rye and his Conference Committee hosted a
fine educational experience. The meeting was at a very nice conference
facility on the campus of Lewis-Clark State College. There was a small,
but enthusiastic, crowd of attendees (about 50) from across the area (I
talked to people from Mt Pleasant, UT and Ontario, OR as well as Boise,
Twin Falls, Coeur d'Alene and, of course, Lewiston). Vendors of equipment
and services were well represented with 17 displays. |
| I will list a few of the presentation
high points from my point of view. The program led off with Dennis Bing,
who works for Drager Medical, but is a fully qualified and experienced
respiratory care practitioner. He gave a good review of the methodology
and utility of expired carbon dioxide monitoring in critical care. Dennis
presented an impressive list of the possibilities of the technique to include
ten very useful functions. The list includes confirmation of intubation,
assessment of ventilation, assessment of increasing airflow obstruction,
efficiency of CPR, assessing pulmonary perfusion, detection of pulmonary
embolism and ventilator management by determining deadspace to ventilation
relationships. Dennis feels that routine use of the technique is cost effective
and the currently available systems are reliable and easier to use than
earlier renditions. |
| AARC House of Delegates Speaker,
Denise Johnson from Minnesota, gave an overview of the current challenges
and opportunities the profession faces with particular emphasis on patient
advocacy. |
| Kristin McFall works for Hill-Rom.
She gave a presentation titled “Bronchial Hygiene”. I definitely expected
an hour-long lecture on “The Vest”. Well, Kristin is a salesperson for
“The Vest”, but she also has CF. She uses “The Vest” as well as all the
other bronchial clearance devices and techniques. She related fascinating
personal accounts of her own therapy and the thought processes of a patient
with this frightening diagnosis. The audience was thoroughly absorbed and
asked countless questions at the conclusion of her discourse. |
| Dr Henry Covelli is a pulmonologist
practicing in Coeur d'Alene. He has a very informative, but relaxed, presentation
style. He discussed new concepts in COPD therapeutics and patterns of PFT
results in two separate lectures. Dr Covelli had a great idea for maintaining
participant interest in the last two presentations of the meeting. He gave
a brief quiz following each talk, and the attendee with the best grade
got a $50 bill. |
| Other presentations covered avian
influenza, neonatal and pediatric emergency care, ventilatory recruitment
maneuvers, APRV and the experiences and perspective of a respiratory home
care patient. |
| President Rye made sure that the
conference was not all learning and concentration. He added in an evening
of socialization and relaxation at a Lewiston watering hole, Boomers Sports
Cellar. A large number of the participants and their spouses enjoyed conversation,
food and drink. As the evening progressed, a Karaoke system appeared and
nearly all of your colleagues in attendance were eventually swept up in
the excitement and performed. |
| The ISRC Annual Business Meeting
was also included in the conference. The minutes of that meeting are included
in this newsletter. |
| I want to thank President Rye and
his committee for a very well produced conference. |

ISRC is still working to develop
a database of volunteers--Idaho RTs willing to go on record as willing
to help in the event of an emergency. There is already a form to fill out
on our web site. Just go to disaster_response_application_form.html.
In addition, it would be helpful if anyone that is or has participated
in any emergency preparedness programs to let me know about these activities...what
is you facility or community doing to prepare and what capacity you are
involved. This information is kept by the President of ISRC. Please e-mail

| My comrades and I have recently
recovered from the toughest job we've ever enjoyed. |
| Asthma camp for young asthmatics
ranging from 8 years to 13 years old was held August 7th-11th with the
gracious hosting by the Methodist camp and generous sponsorship by Saint
Alphonsus and the American Lung Association, among others. A good
45 minute dirt road drive north of Fairfield, Idaho, into the southern-most
part of the Sawtooth mountains put us somewhere beyond Soldier Mountain
ski resort. |
| Thirty two young people came from
western and eastern parts of the state some of whom had never been away
from home. There were tears of course, and well printed in my mind is the
picture of dad on departure day with his hand on the glass window of the
bus where his son was sitting. I later did my best to assure him
with the knowledge that the team of doctors, nurses, respiratory therapists,
and experienced teen counselors would see to his son's safe camping experience
and he would have a good time to boot! |
| Initially, we staff members
(some of us newbees) were feeling, "What have I gotten myself into?" |
| These were special children with
special needs and from, to us, an unknown variety of backgrounds.
But when one child told me she had never been fishing and I just helped
her catch her very first fish, I then realized what a privileged moment
I was allowed to be a part of. By Friday afternoon when it was time
?all so soon? to begin the return trip home, I got an inkling of what that
dad had felt 5 days earlier. |
| Those 5 days were well spent getting
these young people to participate in activities they may have never thought
was within their abilities. Many, "I can'ts" were seen doing things
they had never done before. Being amongst peers with asthma who were
charging ahead gave some the courage to overcome fears that plague many
asthmatics while the health professional staff cheered them on. |
| The professional staff also shared
in asthma education. |
| Amazing how little asthmatic children
know and understand about their own chronic illness! What fun we
and they had as different staff members delivered asthma education in creative
ways that helped them get involved in their own learning experience.
Already, some of us have met again to plan next year's education and activities
at Camp Super Breathers with a proposed theme of "Wilderness Olympics." |
| Lord willing, we'll get the privilege
of doing this again next year...if more staff would join us the experience
for everyone would be all the better and we all could recover from this
toughest job we've ever enjoyed all the sooner. |

1, 2006. She currently is undergoing treatment in Los Angeles, CA.

| The ISRC has a rich history in
the AARC Sputum Bowl competition. We have had four national championship
teams over the years since the Bowl's inception in 1978. After several
years of non-participation, I am happy to announce that our state will
be represented in this year's 29th annual contest at the AARC International
Respiratory Congress in Las Vegas. The team of four Boise State University
faculty members consists of Jeff Anderson, Lonny Ashworth, Jody Lester
and T.J. Wing. |
| Since 2004, a Student Sputum Bowl
has been added to the contest. We are sending our first competitors to
this year's event. The team consists of BSU students Lacey Matteson, Rebecca
Brollier, Denise Chase and Eric Peart. |
| Preliminary rounds are Monday and
Tuesday, December 11-12. If you are attending the meeting, please stop
by and cheer for our teams. Finals are Wednesday night, December 13. |

Editor's note: I think we all
know that pulmonary rehabilitation works. But government funded insurance
has not yet accepted that. Apparently, due to this lack of insurance reimbursement
for many patients, no healthcare organizations in the southwestern Idaho
area other than St. Alphonsus RMC offer this service. St. Alphonsus graciously
sponsors this program for those under-served by their insurance. I asked
Michele to give our readership a description of the program.
| Pulmonary Rehabilitation was begun
at St. Als in 1990 with our first patient beginning in May 1990.
It was initially run by Michele "Lancaster" Andrew, Carmen Griggs and Jan
Gross. While our numbers have grown considerably, the program is
run essentially the same – 18 sessions of Phase II (new out-patient) plus
8 education sessions – then Phase III (maintenance) for at least a year. |
| Several years later we began a
Cardiac Rehabilitation program as well. Through the last 14 years,
Cardiac Rehab split off, then two years ago was reunited with Pulmonary
Rehab due to staff and space considerations. Currently, the programs
are run by Michele Andrew as coordinator, RRT and exercise physiologist;
Jim Baker, RN as Cardiac nurse and Debbie Russell-Smith (a nurse from England
who is working on her National license) as exercise tech. We moved
from the main campus of Saint Alphonsus to the Meridian Health Plaza in
June 2006 to accommodate the new construction. We hope to move back
to campus in several years, leaving a satellite facility here. |
| We have 22 Phase II Pulmonary patients
who generally exercise and go to education sessions on Tuesday/Thursday;
26 Phase III Pulmonary patients who exercise either T/Th or M/W/F; 14 Cardiac
Phase II and 26 Phase III who generally exercise M/W/F. Of course,
we exercise our Phase II Cardiacs without the Pulmonary patients per Medicare
Guidelines. |
| Pulmonary Diagnoses are generally
COPD and IPF but I've had some strange ones or unusual ones over the years.
Scleroderma and pulmonary hypertension are two more common ones; ARDS recovery,
Rheumatoid lung, other collagen-vascular diseases with pulmonary manifestations
and even a catamenial pneumothorax (look THAT one up). |
| Medicare just increased our covered
Cardiac Diagnosis to include valves, stents, PTCA, and transplants.
While CHF and cardiomyopathy are still not covered we have a grant to help
pay for these patients as we see great improvement and have even gotten
several patients off the transplant list by managing their diet and exercise.
We offer 36 monitored exercise sessions and 8 education sessions for Phase
II, then 1 year Phase III. We recently have been working with Dr.
Graham Wetherley on his EECP (enhanced external counter-pulsation) patients. |
| As far as coverage: Medicare
covers Cardiac Rehab for their specific diagnoses. We still fight
with Noridian (the new Medicare Fiscal Intermediary) for coverage of Pulmonary
Rehab and I do have scholarships for them as well. The kicker is
that True Blue and TriCare as well as the VA pay for Pulmonary Rehab.
Some Private insurances are great at coverage but others specifically prohibit
both Cardiac and Pulmonary as covered items. |
| All in all, we feel we have a successful
program but there is always room for improvement. I especially want
to thank Dr. Kate Sutherland, Dr. Bill Dittrich, Dr. Sogol Nowbar and Dr.
Saadia Akhtar for their continued support and patient advocacy for Pulmonary
Rehab. |

MEMBER GETS AARC AWARD
|
| Robert “Bob” Roselle, RRT, CPFT
will be receiving the AARC Management Specialty Practitioner of the Year
Award at the Awards Ceremony at the AARC International Respiratory Congress
in Las Vegas. Bob is currently the Director of Cardiopulmonary/Neurodiagnostics/Sleep
Center at St. Joseph Regional Medical Center in Lewiston. He has been in
the field for 31 years and in management for 29 years. Bob started his
RT career in California, then went to Oregon, and has been at St. Joseph
for 19 years. He has been a Delegate for the ISRC and served on the RT
Licensure Board for the Idaho State Board of Medicine. Congratulations
Bob. |

| President Chad called the meeting

| Treasurer's Report: Carrie
was unable to be there. Chad reported that we have $5660.00 in the
checking account; this does not include the conference costs or deposits. |
| PACT: Con was unable to
go this past year. He stated that home care agencies need to get
letters from us and patients to ask them to send legislators letters to
support HR 964, which is reimbursement for respiratory care in the home. |
| Committee Reports: |

Membership: The ISRC pledged to add
10 new members this year
-
Delegates: Carla reported on the summer
forum. The AARC disaster relief team needs 50 members, and is having
a hard time getting enough people to sign up for it. You get paid
transportation and wages, but your job may not be there when you get back.
There were two resolutions discussed,
#1 To develop a list of speakers, willing to come to annual meetings to
speak. #2 Training paramedics on ventilator management in the hospital,
this was changed to use during transport only.

Newsletter: Tom will be sending one
out with a recap of the conference.

Nominations/Elections: Kelly Dwello
is the new delegate. Nominations needed for next year are President
elect, Delegate, Treasurer, and Board member at large.

Asthma Camp: Michelle reported that
they had 32 kids this last year. They always need more volunteers
and can take more kids 9-13 years of age. There is a $25.00 fee to
apply, but the camp is free.

New Logo: We have a redesigned
logo for the ISRC. It will be put on letterheads, etc.

The present ISRC conference has been
good. It was decided to ask Coeur d'Alene if they would consider having
it next year.

Concern was raised about having a more
controlled process for the balloting process, such as an identifier on
each ballot.
-
Sputum Bowl: It was discussed about
restarting a sputum bowl; participation has always been an issue. The BSU
students want to have a team.
-
There have been three applications
for the scholarship.
-
The next meeting will be in Twin Falls,
on November 3, 2006 at 7:00 pm, place to be announced.
